Be on the lookout for two emails from the Practice Better client portal. The first will prompt you to activate your account, and the second will contain links to the intake forms you will need to complete before your visit.

Intake forms need to be completed at least 24 hours prior to your appointment.

Feel free to upload any labs or medical records to the ‘Documents’ page on Practice Better prior to your appointment for me to review.

If your appointment needs to be canceled or rescheduled, please do so with at least 24 hours notice. If you are unable to make it to your scheduled appointment, your deposit can be applied towards a rescheduled visit for up to 1 year. After 1 year, a new deposit will need to be made to secure your Initial Consultation appointment.
